It's only missing if one fails (or refuses) to look.

to a happy ending. A PDF copy suitable for printing
is also available.

Congress also held extensive hearings on this subject
approximately six months ago. I'll link separately to
a study from CSIS on the broader scope (pre-war, war,
and post-war).
